We are looking for volunteers who are friendly, empathetic, reliable and could  provide practical help for vulnerable adults of any age to come home after a spell in hospital.

The Derbyshire Home from Hospital service assists vulnerable adults of any age to come home after a spell in hospital. It also assists those who are living at home but are at risk of being admitted or readmitted to hospital.

The help of a Home from Hospital Volunteer can be invaluable in settling people back into their own homes by

  • supporting with small tasks such as shopping and/or prescription delivery
  • making sure the home environment is warm and comfortable
  • arranging for key safes/community alarms/telecare systems to be fitted if required
  • supporting people to pay bills and make appointments
  • signposting people to transport options
  • simply being a listening ear.

This is a short term service for up to six weeks to help prevent hospital re-admissions and to help people get home sooner.

Volunteers can offer support on a weekly, fortnightly or ad hoc basis, depending on what help someone needs and volunteer availability to support them.
